There are only a few reported cases of anal canal leiomyosarcoma and around 300 reported cases of rectal leiomyosarcoma. Diagnosis is virtually impossible without proper immunohistochemistry. Prior to the application of immunohistochemistry to their diagnosis, leiomyosarcomas were misdiagnosed as gastrointestinal stromal tumors, which have a different treatment approach and prognosis.
