The same optimal nutrition is required when the patient is discharged and sent home. In-home nutrition also plays an important role in preventing the readmission of the patient within a short period of discharge. Transition from hospital to in-home nutrition therapy plays a crucial role in maintaining the health of the patient.

Transition phase is the subsequent part of the stabilization phase and usually lasts for 2-3 days. The transition phase is intended to ensure that the patient is clinically stable and can tolerate an increased energy and protein intake. Transition phase is followed by rehabilitation phase. The aim is to promote rapid weight gain, stimulate emotional and physical development and prepare the patient for normal nutrition at home.

The knowledge about nutrition therapy in transition is not only important for nutritionists but also important for life science graduates and other medical professionals who deal with medically compromised, ICU patients and general patients.
