Acute colonic perforations are very common surgical emergencies encountered by the general surgeons in the emergency department. Colonic cancer, diverticular diseases, and inflammatory bowel diseases are common causes. However, colonoscopic perforations are also important causes. When a patient presents with peritonitis features such as guarding and rigidity, they may be in shock also. Few scoring systems have been given which will detect the severity of peritonitis and can predict mortality and morbidity also. Diagnosis should be made early based on clinical symptoms and signs. A chest x-ray is enough to make a diagnosis of perforation detected by pneumoperitoneum. CECT scans can be done in selected patients. Timely resuscitation, operative intervention in the form of closure of perforation, and drainage of the peritoneal cavity reduce mortality rates and morbidity. Resection and anastomosis and Exteriorization – Hartmann’s operation and other details are discussed.
