Intercoastal insertion is one of the commonest procedures in emergency departments performed by emergency care physicians, interns, resident doctors, etc. Although the insertion of the intercoastal drain is a common practice in the medical field, inappropriate insertion may lead to complications. It is generally performed in emergency conditions and may cause morbidity and occasional morbidity when not inserted correctly. This is mainly used in cases of collapsed lung, lung infections, hemorrhage of lungs, breathing difficulty, cardiac, and lung surgeries. During intercoastal drain insertion, a hollow plastic tube is inserted between ribs into the pleural space. The tube may be connected to a machine to help with the drainage. Proper placement of the intercoastal tubes can help in effectively evacuating air, fluid, and blood from the injured part. The use of image guidance such as ultrasonography should be considered in cases when fluid or air cannot be aspirated with a needle at the time of anesthesia and especially for cases of empyema and effusions.

Knowledge of anatomical landmarks, the position of the patient while tube insertion, indications, advantages, disadvantages, contraindications are very important for a health care provider to effectively place the tube in an appropriate position and avoid any unnecessary complications.
